OFIFC Life Long Care Programme
About the Life Long Care Programme:
The Life Long Care Programme (LLCP) provides a wholistic approach to Long-Term Care (LTC) community support services that incorporates the physical, mental, emotional, and spiritual aspects of life. Through this approach the LLCP works toward enhancing and improving the quality of life and living conditions for urban Aboriginal people requiring LTC community support services.
Goal
The goal of the Life Long Care Programme is to provide community support services to urban Aboriginal people who are physically disabled, chronically ill, and/or who are frail elderly. In addition, the Life Long Care Programme strives to ensure quality of care and appropriate support systems for their caregivers.
Client Eligibility
A potential client must meet one of the following criteria:
Applicant requires assistance with the necessities of daily living because of mobility issues (physically disabled, requires assistive devices, amputee, etc…).
Applicant requires assistance to ensure their health and well-being because of complications arising from long-term illness, disease or multiple health issues.

Applicant is frail elderly, vulnerable and/or whose wellbeing is at-risk.
Applicant is suffering from severe debilitating health condition(s) that limits their independence (cancer, hemodialysis, palliative, stroke, etc…).
To determine if you and/or your family member are eligible to be a Life Long Care (LLC) client, please schedule an intake meeting with your community LLC Care Support Worker.

Services
Currently, the Friendship Centre Life Long Care Programme is able to offer up to twelve (12) of the following LTC support services within their urban Aboriginal community.
